Activity Description

Croup is a common respiratory illness that affects the trachea, larynx, and bronchi, leading to inspiratory stridor and barking cough. This infection is a self-limited disease typically observed in children younger than 5. The clinical spectrum includes laryngotracheitis, laryngotracheobronchitis, and laryngotracheobronchopneumonitis. Most children with croup improve with a single dose of dexamethasone and supportive management. However, a few with severe symptoms require racemic epinephrine in addition to dexamethasone. Uncommon complications include bacterial tracheitis, pneumonia, pulmonary edema, and, rarely, death. This activity reviews the evaluation and management of croup, emphasizing the importance of the interprofessional team in improving care for patients with this condition. As an upper respiratory condition, stridor is the presenting symptom, which must be differentiated from wheezing caused by lower respiratory conditions. This activity emphasizes the importance of distinguishing between upper and lower respiratory signs and symptoms, as this differentiation guides appropriate management. The activity also discusses the differential diagnosis of croup and its management, highlighting the need for prompt differentiation of conditions, especially severe and rapidly deteriorating conditions such as epiglottitis, which require intensive care admission and actions to secure the airway.
