Activity Description

Hypertrophic scars remain confined to the site of injury and may regress over time, whereas keloids extend beyond the original wound borders and persist without regression. Histologically, hypertrophic scars exhibit a wavy, organized collagen pattern, whereas keloids display irregular, disorganized collagen deposition. The development of these scars results from dysregulated wound healing influenced by genetic, cellular, and environmental factors. Advances in imaging modalities, including high-frequency ultrasound and laser speckle contrast imaging, have improved diagnostic accuracy and treatment planning. Prevention, particularly for individuals at high risk of keloid formation, is emphasized with prophylactic measures aimed at minimizing fibroblast hyperactivity.

This activity provides a comprehensive review of hypertrophic scars and keloids, focusing on their pathophysiology, diagnosis, and management. This activity covers both current and emerging treatment strategies, including corticosteroid injections, laser therapy, radiation, and newer approaches such as monoclonal antibodies, photobiomodulation, and targeted molecular therapies. This activity provides clinicians with the knowledge to improve clinical decision-making and patient education, as well as implement evidence-based strategies for managing hypertrophic scars and keloids. In addition, this activity highlights the importance of interprofessional collaboration among healthcare providers, including dermatologists, plastic surgeons, and primary care providers, to optimize patient care and outcomes through early recognition, appropriate treatment selection, and ongoing monitoring.

At the conclusion of this activity, the learner will be better able to:

  • Identify the clinical features and risk factors associated with hypertrophic scars and keloids to facilitate early diagnosis and intervention.

  • Implement evidence-based therapeutic approaches, including corticosteroid injections, laser therapy, and other modalities, to effectively manage hypertrophic scars and keloids.

  • Select appropriate treatment options based on the patient's clinical presentation, risk factors, and scar characteristics, tailoring interventions to individual needs.

  • Collaborate with the healthcare team to assess long-term treatment effectiveness and prevent recurrence or progression of hypertrophic scars and keloids.

  • Outline the pharmacologic therapy as it applies to Hypertrophic Scars and Keloids
