Activity Description

Complex regional pain syndrome represents a neuropathic pain disorder defined by allodynia, hyperalgesia, vasomotor and sudomotor abnormalities, and trophic changes. Pain is regional, disproportionate to the inciting injury, and persists beyond the normal healing of tissues. The etiology remains unclear but likely involves genetic, inflammatory, and psychological factors. Pathophysiology includes dysregulation of pain processing in both the sympathetic and central nervous systems. Altered neuroplasticity, peripheral sensitization, and central amplification contribute to persistent pain and autonomic dysfunction. Complex regional pain syndrome commonly presents in a distal limb following trauma or surgery. Findings include burning pain, swelling, asymmetry in skin color or temperature, abnormal sweating, and changes in nails or hair.

Diagnosis of complex regional pain syndrome is clinical, based on the Budapest criteria, after excluding alternative causes. Effective management requires an interprofessional approach, integrating physical therapy; pharmacological agents, such as nonsteroidal anti-inflammatory drugs, anticonvulsants, and corticosteroids; and psychological support. Neuromodulation or sympathetic blocks may benefit selected individuals. The prognosis of CRPS is variable. Early diagnosis and treatment improve outcomes, although some individuals develop chronic, disabling pain with functional impairment.

This activity for healthcare professionals is designed to enhance the learner's competence when evaluating and managing complex regional pain syndrome. Participants broaden their grasp of the condition's etiology, pathophysiology, and clinical presentation. Current diagnostic and therapeutic recommendations are also discussed. Greater competence enables clinicians to collaborate with interprofessional teams caring for individuals with complex regional pain syndrome.

Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Apply the Budapest Criteria to accurately diagnose complex regional pain syndrome based on clinical and diagnostic features.

  • Implement evidence-based management strategies for complex regional pain syndrome and its potential complications to improve patient outcomes.

  • Improve patient education on complex regional pain syndrome to enhance understanding of the condition and its management.

  • Collaborate with the interprofessional team to educate, treat, and monitor patients with complex regional pain syndrome to improve health outcomes.
